Perhaps base this off of the feat in the WoT book called Heroic Surge, it would be based off of character level per day (1 + 1 per 4 or 5 levels) Only able to be taken once. I might also add a prerequisite of Improved Initiative or something along those lines (maybe) to emphasize the mastered reaction time.
Flying leap. Prereq - heroism, speed 40 or less.
You move farther than you normally could for one action.
When executing a charge or partial charge, your base speed increases by 10 ft plus 5 ft for each point of Cha bonus, if positive. At the end of your action, you are prone until your next initiative even if you can stand from prone as a free action.
Don't like this one because of your prerequisite of speed 40 or less, it's the same as benefiting a character with a low ability score because of it being low. And ditch the heroism requirement, I could see any character being able to do this. Make it usable once per day, plus once per point of Con modifier and make the bonus a flat bonus, like half your normal base movement. So for a base move of 30, you'd get +15 or something of the sort. Also, the whole prone bit I could see for the "leap" but you mention nothing about leaping in the feat so I'd drop the prone affect, and add the penalty of being flat-footed till your next action like Heroism.

You interpose yourself between an ally and danger.
When an ally is struck by a melee, ranged or area attack which you are not also subject to, you may make a grapple attempt against the ally. If successful, the ally takes no damage from the attack and you take maximum damage.
Again drop the Heroism prereq. Add these requirements, ally must be within your reach and you must not have acted yet this round or you must be able to act (i.e. heroism). Touch attack against your ally. Same results.
Those are my thoughts on those three, I do agree with you about having heroic feats and a lot of times I'll let a player perform some heroic action if it hits it up with the plot without a feat or whatnot. But that's usually for flair and heroic effect.
