I think some of those situations you guys mentioned (the bridge, the table) illustrate the reason they DIDN'T put in rules concenring high ground. It's too situational. Sometimes it would make sense getting an advantage, like if you the archer on the cliff or guard tower, but other times it just sucks like if your trying to fight standing on a table or getting skewered from under a bridge. Best to leave it up to the DM and rule according to the specific situation
