Like a lot of others that are saying I tend to have both. I tend to start with a published thing like a box set and then add more locations and rumors and NPCs. Then these come out more as the box is running out of levels and we continue for a while where I might add more published things.

My current campaign started off with the Stormwreck box where we ran mostly with the published adventure and added a few side treks that were homebrewed. When the Pcs wanted to leave, they ended up with the published adventures after Icespire Peak for several levels, again with some side quests and added town elements. That is finishing up and I plan to take the final mission and tie it back to the starter box for a final showdown.

A mix, I voted "Premade", because I tend to use published material but I am no respecter of setting or lore. For setting I will use the maps, maybe the gods, maybe some NPC, some lore and maybe some other lore from somewhere else. Adventures, I use more of, but the players go off script then the adventure is likely to be beaten with a hammer (or some stuff from a different module get bolted on) until it makes some bit of sense.
 

Premade campaigns. My ideal is a mashup of multiple similarly themed campaigns to give more options and paths to explore.

If d rather focus on bringing an existing world to life rather than coming up with the details or winging it.
 

I do a mix of both: however I often customize pre-made campaigns to suit my (and my players') tastes.

I really hope some day of running a campaign in my custom setting, but I have to find suitable work arounds for otherwise restricting certain classes and species. Players don't like that, so I need to find a compromise (eg, Tieflings in their "default" form have no place in my setting... but people love them tieflings so I need to work something out).
