How Dark Adept Should Be Notated

I've changed all the stats in order not to violate the NDA. Specifically, note the details of the stats, attacks and magic. The question marks indicate I didn't understand the rules or there were none. Please don't make me look-up each power for each monster all the time. Just notate the power in the bestiary or module.

Dark Adept
human male or female
lawful evil
AC19(+5 chain, +2 heavy shield, +2 dex)
hp17(con13+HD 1d10)
str 14(+2)(+1 toughness training)
con 13(+1)
dex 14(+2)(+1 dodge feat)
int 13(+1)
wis 13(+1)
cha 13(+1)
speed 25ft.
melee attack mace 1d20+4(+2 str, +2 from mystery feat) vs ac 1d6+2(+2 str)

can use 2 powers or 1 power twice during the encounter

cure light wounds(ranged touch/standard action/heal 1d8)

command (ranged 30ft.? your wis vs target's int? force target to grovel 1 round, grovel condition? attack-2?) rolling a 1d20+1your wis mod is the same thing as making the target roll something versus DC11.

unholy smite (encounter x1) on a hit do an extra 1d8 damage

no to attributes, no to twice per encounter, but yes to abbreviated spell descriptions.

I like the Pathfinder style notation of AC and think that feats should be notated to the right of each relevant ability score for the same reason. It does look a bit cluttered. But 4th edition had a few character sheets that listed skills under each ability very nicely. I'd like to go with that format but take out all the skills and feats that the character doesn't know -- not have a long list.
