How do you note several classes in the stat block?

The standard I think is to keep them separate; I add them up, personally, or don't even list them at all (I'll list "Clr5/Rgr3" and "53 hit points" in personal notes.
 


I usually write, say, "Cleric 5 (Bob)/Ranger 3 ... HD 8d8+8; 46 hp" when I'm doing formal stat blocks.

In my personal notes, lately I've been writing, e.g. "Cleric 5 (Cthulhu)/Ranger 3 ... HP 46 (8 HD)" just as a quick jot. Of course, I've seen that format in the DMG II and in Dragon, so that's kind of influencing my personal notes right now.

I used to abbreviate the classes (Clr5(Fnord)/Rgr3), but some just don't have an intuitive abbreviation (Knight Protector of the Great Kingdom, I'm looking at you!).

This is the format, I've chosen to use. Adding all bonus hp up is simpler than splitting them over every part, not to mention the problem, where to put hit points from Toughness and other sources, if they aren't lumped together.

WotC used to use this format, i.e.:

"Hit Dice: 2d8+2 plus 1d12+1 (18 hp)" [LM, p.106]

And how do you represent any class substitution levels?

The example I'm staring at right now for my own multi-classed character is to possibly take the 6th level Planar Paladin substitution level from the Planar Handbook. The character is currently a Fighter 6 and Paladin 5 and looking to take the planar sub level for a 6th level paladin.
 

I'd note substitution levels in the special abilities after the main stat block. If an NPC is important enough to bother with substitution levels, he's important enough to spend some more space on.
