How does a Gorgon's Trample SA work?

Trample (Ex): Reflex DC 19 half. The save DC is Strength-based.

Is it just a standard action so it has to get within 5' of an opponent and then the PC gets a Reflex save?

Can it "run over" a PC without the PC getting an AoO?

Is it like the Overun and Trample attack option and feat in the PHB?

Could they have choosen a more confusing name for this Special Ability?

The ability is spelt out pretty well on pg.316 of the MM (3.5). Trampling is a full-round action, and the creature can move up to twice its speed.

Can it "run over" a PC without the PC getting an AoO?

The PC has two options - it can take an AoO at a -4 penalty (and take full damage) or make a Reflex save to take half damage.

Is it like the Overun and Trample attack option and feat in the PHB?

Not really.

Could they have choosen a more confusing name for this Special Ability?

Probably. Trample is a pretty accurate description of what the ability does. Though admittedly Grind Thy Enemies Beneath Thy Brazen Hooves also works :)
