Hm. These two sentences don't mean the same thing to me (which is my "fault," though):
Just counting moderate to major errors; those annoying/aggravating mistakes that make you cringe, curse, or roll your eyes.
My previous answer is based on the bolded part: Things that make me cringe, curse, or roll my eyes.
I have very little tolerance for mistakes I make, though, even small ones, so I get mad at myself over very little.

If I answer to the spirit, rather than twist the letter, of this poll and look at how many moderate or worse errors I really make, then the answer is none.
 

Every now and again... I'd say between 0 and 1 per session. That relies on me having time to check up on the rules I intend to use before a session. Might rise slightly if we end up heading off in an unexpected direction.

Had a few long term glitches switching from 3.0 to 3.5, assumed some things stayed the same, when they had changed. At least they were consistent, if nothing else.
 

Hm... if forgetting a +1 every now and then doesn't count, I guess it is something like 1 error every three or four sessions. The last very big one was giving power attack to an incorporeal creature (which can't meet the STR requisite). Luckily, I noticed it after the first time it used the feat, so I retconned the attack and changed the feat to something else.
 

Enough...

Most of them are small, stupid checks that don't really matter, but sometimes I forget just the most basic information.

Then there was the time that a chasme demon used multiple rays of enfeeblement to reduce the wizard to Str 0. Nothing bad really happened, but still...
 

Isn't the problem with this poll that you are more likely not to realize you've made a rules gaffe? I mean if you think you know the rules, you'll answer none. But what if you are wrong and make mistakes all the time?

This poll may as well be how many invisible creatures did you see today?
 

Difficult to say.

I suppose that our group makes a few, but then there are a lot of rules we also decide to gloss over, as this fits our gaming style. From an outsiders POV it would be about 11+; from an insiders POV probably 1-2, often 0.

And as for Really Major Mess-ups, well, those would either be re-adjudicated immediately or patched up in our on-line correspondence.

But this problem is "solved" by another problem with the poll: You can't vote 'none.' ;)
 

IMO, all rules are subservient to the decisions of the DM. If there's a conflict between A) the rules, B) fun, C) Story, or D) pacing - then A loses every time. The rule is a suggestion, though I tend to keep the rules the *characters* use consistant. For instance I won't change what a spell does in the middle of play when a PC casts it. But there may be other effects and consequences of that action, depending on B, C, and D above.

In other words, I break no rules. There is no spoon. :)
