I do the following for my campaigns:
- I have all of my monsters printed up on 4x6 cards (no book flipping), which is a big time saver and it makes it easy to mix and match for encounters
- I keep those cards right in front of me and roll in the open
- I have a player track our initiative cards
- Each player is responsible to track the conditions they place on their enemies (in addition to their own conditions as well); this is a huge bookkeeping timesaver for the DM and it really forces the players to pay attention (example: if they fail to remind me that a monster is slowed when the monster acts and I move normally [because I forgot, I wouldn't do it on purpose], it's on them)
