Do many DMs really let players get away with this kind of abuse? I might let someone get away with two prestige classes, particularly if one of them was a five-level class, or one of the “dual-class” PrCs.

But I’d never let a player gain a third unless they had a really, really good in-character reason. (And I really can’t think of any such reason.)
I allow three Prestige Classes, sure. But I won't allow Impulse Boots, so I don't allow that particular combo of PrCs I posted above (which incidentally offers 9th-level Wizard spells, 8th-level Druid spells, +14 BAB, incredible saves, and good skill points).

Now, if they wanted to do Wizard3/Druid3/HarperMage1/MysticTheurge1/Rogue2/FochluchanLyristX, I'd be fine with that.

As I said, these are just personal quibbles, not enough to make me dislike the book, or not recommend it, just little things that bug me.

The pictures in the items section all show one of the items 'in use', thus a caption telling you what that item is, helps you to look it up.

Yep, taste. Subjective. Just MoI doesn't fit mine. And the worst of the lot was the "Sniff out foul magics" bit for one of the prestige clases. I hadn't heard a line like that since the 60's Batman series or the 80's Justice League. It was trully a bad line.
