Epic shmepic. A 1st level sorcerer's toad familiar could hit a Force Dragon. All it has to do is roll a natural 20.

And the point remains that most real 40th level PCs that actually advanced to Epic levels instead of being created as 40th level characters, were role-played and not roll-played, and didn't have some twinky combination of classes couldn't hope to touch this thing.

CR is a steaming pile of doo, especially at Epic levels.

Oh, yeah, Metalsmith: I don't believe Brilliant Energy affects natural armor.

DM: "Uh, okay. The force dragon gets an attack of opportunity - clatters dice - oh, that's gotta hurt. Hoppy just got swallowed whole. You hear his little voice in your head for just a few seconds before his life force is completely extinguished. Lose that con bonus, eh? And now the dragon looks at you, horrified, and says 'You would do that to your best friend?!?' with righteous indignation in his voice. He's breathing on you; roll a reflex save."

PC: "HOPPPPPPY!"

This is when you cast that harm you had stored up, through the toad, into the dragon's belly.

+30 BAB, +1 Weapon Focus, +2 Epic Weapon Focus, +10 Strength Bonus, +10 magical Item. That's +53 and I'm not even trying.

Building on Crothian and Victim... and knowing that this character (at level 40) has money to burn on the unusual (and more expensive) bonuses...

Pure Class Fighter, who put some advances into his Con:
Starting Strength 16. +3 Level (non-epic), +3 Level (epic), +5 Tome of Strength, Strength + Con Bracers +12. One addition of the feat that adds 1 to Strength. That adds up to a 40 Strength.

+30 BAB, +1 Weapon Focus, +2 Epic Weapon Focus, +10 Magic Sword, +15 from Strength, +10 Luck Bonus, +10 Competence Bonus, +10 Insight Bonus, +10 Sacred Bonus, +10 Morale Bonus.

That's not unreasonable for a fighter who is concerned with hitting what he faces in combat. That works out to +108. So you can Power Attack for 25 and only miss on a 5 or lower. Or power attack for 21 and only miss on a 1.

These are EPIC levels. The fighter had darned well better KNOW it's hard to hit things at these levels and plan for it.

On a side note: Druid with a Flame Blade (energy substituted if necessary) will basically always hit (it's a touch attack). Since the spell has a variable, numeric portion (and these are 40th level characters)...

The druid has 4 Bonus Feats and 7 feats from levelling up. So... take one Improved Metamagic and four Improved Spell Capacity feats. That (relatively modest) combination lets you cast an 11x Empowered Flame Blade which does (1d8 + 10) x 5.5 per strike. That's 60 to 99 damage per hit for up to three attacks per round for 40 minutes. This can be made nastier.

Naturally, this approach requires you to punch through SR. If you can't get through the SR, though, you have much bigger problems than how to hit the dragon in melee.

Man, I like that imagination.
If nothing helps, look at the Sultans of Smack. There are some very simple combos, too (not that with five classes, maxed out items and abilities...).
