Although already mentioned by the article Umbran linked to, it should be noted again that any significantly large explosion can create a mushroom cloud. It is not particular to nuclear weapons.

Well consider that nuclear weapon testing has been done underground for decades...

Either something went horrible wrong, or it was something else.

But only if they definitly do not have A, B or C weapons. And if they do not have any, it is far more convenient to let the world believe, that they do have them. (although, I am sure that there are lots and lots of satelites monitoring any movement of any country on the verge of getting a-bombs).
