D&D 5E Lethality in 5e: what is your preference and how do you achieve it?

[MENTION=6788507]Mondas711[/MENTION] - your post made me think about how some of my players react to going unconscious. Many of them outright fear it and think of it as death. I wonder if that's because they had experience with OD&D or if it is just pride?

Maybe they just don't like going a single round without doing something?

Well, I use two ways to achieve more lethality,

And I also use a custom table for sustained injuries every time players fall to 0 hitpoints, or take massive damage (equal or greater than half), explained here: http://hedgegm.blogspot.hu/2015/01/lingering-injuries-and-wounds-massive.html
