Kacdan will take the AoO with a bite, probably not knowing any better. Let's see how that resolves before Kacdan decides what to do next.

Kacdan takes a free action to pick his jaw up. Plane shift as a free action as a supernatural ability at distance requiring a reflex save?! "Stutter...stutter..."
Kacdan hovers and casts shield of faith defensively. Let's hope he survives to next round.

Conceivable. I have no choice but to agree with the second part of Kacdan's assessment.

Kacdan has some more protection.
The protection does not help, as John smacks Kacdan (rolled 41) for 32-10=22 (I forgot DR last hit, so I subtracted it twice this time.) points of damage. Kacdan feels tiny parts of her body disappear, (rolled 21 Fort save) resulting in 5 points of Constitution drain. John continues flying to 70N35E50Z, northbound. Kacdan tries biting John as it passes, but bites a place that disappears as it is about to be bitten (rolled 33, 15% miss).
A window in space opens up near Kacdan, who manages to get out of its way (rolled 27 Ref save). The window disappears.
Kacdan has 106 HP remaining. Her Strength is 30. Her Constitution is 16.

Kacdan growls and hopes for some more luck next round. He breathes another gout of flame at John and hovers up to 0N0E110Z.
Edit: I've never wished a creature had resistance as a 0th-level spell more than I do now. That would be more useful than any other spell in his current repertoire.

John fails to dodge the fiery breath (rolled 17 Ref save), even though it is a tad easier to dodge now than it was last time John was breathed on. Kacdan burns John for 53 points of fire damage. John has 195 HP remaining.
Kacdan can breathe next round. I would have said this was a wasted roll of a 1 on a d4, but Kacdan cannot use her Recover Breath feat anymore due to her lowered Constitution.

Kacdan breathes another gout of flame and then hovers directly east to 0N100E110Z. John sure has a heck of a flight speed, probably almost as good as Kacdan's.
John does not have a good flight speed, but John is taking double moves, and I messed up. 
John should have been at 50N20E80Z. Kacdan's breath still reaches John. Let me know if Kacdan would have moved somewhere else.
John cannot dodge (rolled 18 Ref save), taking 46 points of fire damage. John has 149 HP remaining. Kacdan can breathe again in round 657.

A bank of thin mist surrounds Kacdan, as do several swarms of locusts that get together to form a really big swarm.
Kacdan resists the effects of the mist (CL check 29, Will save 33). She does not know what the mist is.
She recognizes the locusts as an insect plague. She is bitten by two of the swarms for 6-5+7-5=3 points of damage. She is not nauseated by the swarms (worst Fort save roll 17). Kacdan has 103 HP remaining.
