How Would YOU Create a Lycanthrope Player Character?

Numerous iconic abilities that are the focus of the character, that progress and strengthen over the course of the game? That maps to the class mechanics.
 

I have a similar PC in my campaign, a dhampir struggling against an urge to full blown vampirism.

We handle it using a variant of the VRGtR Dark Gift Second Skin. Instead of just changing form, the change switches the PC to an entirely new character sheet (lycan blood hunter, in this case). To discourage overuse, if the PC willingly chooses to make the transformation, they roll their saves against the lycan’s Bloodlust feature at disadvantage.
 






The result is likely something like you become a werewolf but when the moon is full you kill all your allies. Which of course the people who want to be a werewolf will always say something like they'd be the ones in control because they've never watched an old school werewolf movie.
I am not able to predict the future. So I will wait and see.
 

I would go the opposite way of this DM: either I wouldn't allow it, or I'd have it be an actual curse that afflicts the character periodically/monthly. While I understand the lack of "lupine" options for PCs in 5E, lycantropy is supposed to be a serious curse that can forever change a character.
