Lycanthropes.

For those that want infected werewolves, I'd make this change. If they haven't cured their disease by the next full moon, they get full on cursed lyncanthropy. That gives you time to construct what the player's stats would be while cursed for the next gaming session.

I really like this. If ever I need infected werewolves in my game, this is how I'll be doing it.

Well, that or "infect lots of people with moon fever, and the few that survive get kidnapped for our werewolfism ritual."

I also like the idea of the afflicted lycanthope as a 4e disease that takes some time to process through.

I haven't fully assimultated the rules yet, but at first brush it looks like Sean Reynolds Curse of the Moon rules will be an easy conversion.

I will keep an eye here for a fleshed out 4e disease and will post the conversion once I get a better idea of the new rules and some spare time :)

Yeah...a disease with a special timing, each full moon requires a new endurance check on the track....

Cured / Controlled < Mild Moon Fever (lesser effect) <> Moon Fever <> Severe Moon Fever (night before / after) > Final (curse of lycanthopy, no hope of cure barring a major quest)

With controlled, they essentially become like natural lycanthropes but this "cure" requires special treatment rather than just resiting the curse.
