Really its the population density and support infrastructure, people were never meant to form communities that large. So many people crammed into so small a space does make people go crazy because it's utterly unnatural. In rural areas you see far fewer so insane they can't function in society because their isn't really any support structure beyond family which has very limited resources.

Living in Vermont I know about sparce living...... We are crazy also but its just not as widely reported or dramatized. There is a vermonter that lives on the Canadian border.
