Campaign Cartographer is my choice. You don't really need the add ons to do dungeon or city, but the city add-on can really speed things up.

I also like to use Fractal TERRAINS (not mapper) in conjunction with Campaign Cartographer to make world maps. It makes large, attractive, realistic-feeling worlds very easily. And is fun to play with to boot.
