Medusa-Ghost: When can it use it's breath weapons?

When can a medusa-ghost use her gaze ability? Does she have to be manifested?

Alternate Form works as Polymorph and Polymorph works as Alter Self and Alter Self states.

"You retain all supernatural and spell-like special attacks and qualities of your normal form, except for those requiring a body part that the new form does not have (such as a mouth for a breath weapon or eyes for a gaze attack)."

So, a Medusa polymorphed into a Halfling (or weretiger in this case) could use her gaze attack correct?

I believe that is true. You could of course say that the required body parts for the medusa's gaze is the snakes on the medusa's head and then the medusa would not keep her gaze attack with polymorphed (unless the assumed form had similar snakes), if that is the outcome you want.
 

Also I think it should be noted that as a Monstrous Humanoid the Medusa can not normally be a Lycanthrope since that template is only for Humanoids and Giants.

Correct. It's more applicable to a simple polymorph or a ghost form (manifested?)
