But what if you do the dragon hokey pokey? You put your dragon in, you put your dragon out, you put your dragon in and use legendary all around, you do the dragon pokey and get yourself a rout, that's what it's all about. Inquiring minds want to know.
The way I would do it is if the dragon leaves its lair it loses its fourth LR, but it gets it back if it goes back in. This gives the dragon a reason to retreat to its lair if it’s in trouble.
 

I would let them keep the 4th, because the fight started in the lair. But the chances of them fleeing far enough, but the fight continuing, seems unlikely.

Most Lair effects from the older books seemed to have a mile radius, so I'm assuming you need to get at least over a thousand feet away from the entrance of the lair proper for it to be "out of its lair" mystically speaking. Which is just really far.
 

I like it because it forces choices on the victim's allies (presumably the other PCs). Fight the monster or try and save your team member. If it doesn't do damage, they'll just leave them stuck.
Plus if they have to act to attack the stuck PC, they aren't acting against others
