Mounted Charge w/ Lance

Okay, this may be a silly question, but I'm not sure our group has been doing this correctly...

If I'm mounted with a lance and I charge (or just attack), where do I stop my character - adjacent to the target or 10' away?

We've always placed our character 5' away (as if we impaled the target with the lance) and you just couldn't attack again with the lance unless you moved back 5' (due to the reach weapon). If this is not correct, is it possible to have your mount attack with you when using a lance seeing as it would need to be 5' from the target?

It's based on the range of the weapon. So 10' in your example. Specifically:
Here’s what it means to have a clear path. First, you must move to the closest space from which you can attack the opponent. (If this space is occupied or otherwise blocked, you can’t charge.)
You can't attack your oponent with the lance from 5' away, so you can't charge to that space.

So, you cannot attack with your mount if you charge with a lance then?

A Medium character with a lance {10' reach] on a large mount {with 5' reach] has to stop for the charge's attack before the mount ever has the chance to attack.

I played a halfling paladin with a riding dog. I got very used to the Lance Shuffle...

Round 1 - charge. Attack with lance, finish 10 feet away.
Round 2 - Attack with lance. Dog 5' steps forward and bites.
Round 3 - Dog bites and 5' steps back. Attack with lance.
Round 4 - see Round 2.

On another note, how do you resolve someone's position if the mount they are on suddenly disappears? Let's say someone mounted a gargantuan creature that was summoned in, and was crossing a chasm when the duration expired. Do they appear on one side of the chasm, the other, or in between (falling to their doom?)

I say roll randomly amoung the 4 center most squares of the 20' face of the mount.
