So the "real" challenge of the encounter is once Irontooth is bloodied?

But yes, at that time, I could allow a recharge of encounter powers. I'm not sure what exactly would happen to allow them to suddenly recharge and spend a healing surge, but that sounds like a good idea. It'll keep the action exciting while not being a TPK.

I skipped some of the Encounters in the Keep. There are too many imho.

That's another good point. The keep itself really starts to drag after awhile. Except for the encounter w/ Captain McGhosty (I forget his name) and the kinda-sorta-tie-in to Thunderpeaks, there's not much story that gets revealed by those encounters.

If I were to run it again, I'd skip several of the encounters, including the trap room; that encounter /could/ be salvaged, but it'd be quite some work.

I think most people skip or merge encounters within the keep. There's an excellent commentary (sadly the blog is no longer updated) at Eleven Foot Pole: Keep On The Shadowfell, which highlights the redundancy and repetition (amongst other things). Well worth a look.

Also worth checking out is the excellent 'Orcus Conversion' (of the three 'H' modules) at http://web.comhem.se/mwester/Shadowfell/Documents/H1-H3_Orcus_Conversion.pdf - again, some great ideas and suggestions, plus some lovely print-outs, etc.

YEESSS! That 'Orcus Conversion' is EXACTLY what I've been looking for! I was planning on doing something like that myself, but since that's already made, I'll just as soon use that. I'll use it in a heartbeat. Thank you. And yes, that blog also looks very interesting. I'll give it a look through and see if there's anything that I could use.
