What if Miko was thrown into another plane of existence, just like what can happen when a certain wizard's staff is snapped? And there is only one planar traveler that I know of in OOTS that could get to her and bring her back.

I'm fairly sure Miko is either dead (explosion, smashing into walls, getting hurled a great distance)

She has no Improved Evasion, nor Wholeness of Body or anything else significantly helpful to her survival at this point.

She doesn't need Improved Evasion unless she fails her Reflex save. A Monk 4 | Paladin 12 would have an 8+dex Reflex save. Supposing she had a 14 Dex, she'd have +10 to her Reflex save, so if it was DC 20, she'd still have a 50% chance of taking no damage.

Belkar can die immediately afterwards; I don't like him, but I like Miko even less, and want to see Belkar live long enough to see Miko die.

It's not a great loss. She wasn't very charismatic to begin with.

I think she is charismatic. She has a big fan club, and Roy was completely smitten with her.

It's only after a while, once the Real Miko gets to be known, that people start to hate her. So, she has raw charisma (creates good first impressions), but a ugly character (and people can't stand her anymore after a while because of the way she behaves).
