PCs die when...

...they don't buy rations.

One time we were chasing some bandits through a desert and the DM kept telling us how many days it took to get from this point to that. Eventually we found the lair after 4 weeks of tracking. The Dm suspecting we were low on food and water asked to see our sheets and found we were low to start with. We had loads of equipment but only one character had any food and it was only a weeks worth of rations. Well we haggled about rangers using their survival skills to keep us alive and it. Both rangers were given a die roll to see how it went. They both failed miserably, one rolled a 1. So the DM decided that we actually perished in the desert and were now undead roaming lost looking for bandits we would never find.

side note: the next time something like that happened it was found that one PC had 365 days of food and water in his backpack. After it was ruled that the guy could actually carry all that food between him and his mount the DM ruled that the PC was poisoned from eating food that had gone bad. He made his saving throw and learned his lesson.
 

Through a willful act. Either through their choice (i.e. heroic or unheroic sacrifice), or through an act of such stupdiity that it might as well have been deliberate.

Player1: the DM would never put anything that powerful in this area. we are only lvl X.


Player 2: i'll be sure to tell your corpse that.
 

...they forget I'm a butthole that's out to get them. :)
...they meet large creatures and forget to run.
...they forget that they've used all their healing spells/scrolls/potions ("becuase the mages still have plenty of spells left so we'll press on")
 

PCs die when

....they fail to ask questions.
....they believe they are invincible.
....players fail to find out what is true and what is a lie.
....they fail to work as a team.
