In previous editions I have read liturature regarding spell mishaps. The most notable is when summoning spells go wrong and a different creature is summoned. In 3.x edition there does not seem to be very much of this, everything happens as planned. Has anyone added similar elements of randomness in their campaign?
Another example of this; in previous editions raise dead was not automatic, you had to succeed at a system shock check. In 3.x there is no risk as long as one can find a sufficient spellcaster to bring a friend back.
