I use my own dieties with names and portfolios stolen from earth (we's gots a lots of gods) since its incredibly easy compared to 2nd edition.
As for the spells, I definitly use the names as printed. This is a hold out from the old days of 2nd edition where travelling from Krynn to Oerth to Faerun is easy either through spelljamming or planescaping. The wizards in the PHB who wrote those spells, regardless of how they may be statted out in a book, are some of the most powerful of any plane anywhere. They're easily powerful enough to travel and many are not afraid of giving these spells away which were then common on Oerth.
Hell, even in the Realms the spells retain their names because of this. Khelben Blackstaff Arunsun even went out of his way to create some rather poor high level spell just to counter Bigby's various "hand" spells due to some spat between them.
I'll use Elminster as an example since most of us know details of him. Take away the all the stuff about being the chosen of mystra, his silver/moon/spell fire ability, and some of his other powers that are directly tied to Faerun. He's still a very very powerful archmage. I wouldn't hesitate to throw a few spells into your "common" spells like those from the PHB with his name on it.
This of course assumes your campaign isn't like Dark Sun for example which precludes most all planar travel possibilities even for powerful archmagi.
