Given that you already have the prerequisites what is the must have/usefulness of

Eyes in the Back of Your Head [General] REQ: Base Attack bonus +3, Wis 19+
Attackers do not gain the usual +2 attack bonus when flanking you. This feat grants no effect whenever you are attacked without benefit of your Dexterity modifier to your AC. Sword and Fist, pg 6.

The problem with this feat is that the characters most likely to waid into combat - fighters, barbaricans, etc are very unlikely to have a wisdom of 19+

Why would barbarians even need this feat? They have uncanny dodge.

This feet has been a write off in our group.

As I look at it, I think it is too weak for any use. I can't see it ever being taken. Anyone with that wis is most likely a caster- and would be better off taking a feat that helped their magic.

So, there are three options-

1) If one raises the BAB and get rid of the Wis requirement, then the feat is probably fine the way it is written.

2) If one raises the BAB and keeps the Wis requirement, then make the feat change to: opponents who flank a character with this feat gain no benefit from flanking(bonus to hit, sneak attacks, etc). Keep the bit about it only working while not denied their dex.

Since the second proposal is rather powerful, I think the BAB requirement should be at least around 13+

3) If one keeps the BAB requirement and gets rid of the Wis requirement, then you could change the feat to the following: at the beginning of each combat round, a character with this feat may select a number of people equal to their Wis bonus that will not receive any bonus to hit gained from flanking this character for the rest of the round.

I like #2 just because there are so few high level feat for combat, but I would go for #3. It is geared for combatants and rewards a decent wisdom.

It is so worth trading a feat for uncanny dodge that I can't believe people are debating it. You can't be a victim of sneak attacks unless you are caught flat footed. If you happen to have the fate domain as a cleric you have immunity to sneak attacks which normally takes 5 levels of barbarian or 6 levels of rogue.

Does no one get flanked by a rogue with +4d6 sneak attack with 2 attacks per round? The feat would stop 8d6 damage or more if the negation of the +2 bonus made one or more attacks miss. This is one reason people buy fortification armor (the other is to stop criticals)

Everyone who qualifies should probably take this feat if they haven't already gotten it for free as a class ability. I struggle to find worthwhile feats for characters if they aren't going the archery route or the maxed exotic weapon route.
 

This feat isn't uncanny dodge. All it does is eliminate the +2 bonus that flanking attackers have to hit you; it doesn't do anything else.
