We average about one combat an hour. Just setting up the markers/minatures/etc., clearing them away, and moving them around slows down our battles way too much. I guess we're willing to live with the approximate visualisations we each hold in our heads. Is it 100% accurate? Probably not. Does it speed up combat? We think so.

No miniatures.

We use one too. I found it an an office depot, I think.

As far as minis go, I never used them once in twenty years of off-and-on gaming, but now I have used them ever since I started playing 3e because the group I joined has always used them. Of course, these are the same guys who taught me how to play Magic:The Gathering. These guys have cost me a lot of money! :)

Wait, now that I think about it, about 15 years ago I played a lot of V&V and Champions with paper counters and a hex map because knocking your opponent back a few hundred feet is absolutely essential in a superhero game! :)
 

We use miniatures (for PCs) and counters (for NPCs) but only in large and/or complicated battles. There´s no point in using them if they just pile over the bad boy and start bashing.

However, when a wizard, 3 rogues, two warhounds, 3 mercenary warriors and his boss mounted on a wyvern attack the party (scattered in and out the walls of the city) minitures ecome invaluable (How far is it? Can I shot him? You said me there´s only 1 of these wolves! Ok, I´ll remind you where´s each one... etc, etc.)
 

Minis are annoying

We get by without minis, and usually without a battlemat as well. I blame it on having very good memory and mental imagery. Oh, sure, it means that each of us has a slightly different vision of what the place looks like, but so far it's been compatible. In extreme cases, we diagram it out on a battlemap with dry-erase markers but we have never, ever used minis or counters. They have the nasty tendency of getting in the way and/or getting lost.

I am surprised some people don't at least use graph paper. My gaming group fully maps out each dungeon meticulously. We always use some kind of designator on the map.
