The Regeneration spell is pretty useless, too.

Silly? Sure.
Let's say your PC are accused of some sort of criminal offense and they are found guilty. Once they get in front of the executioner and they put their head on the block, your 20 level fighter will require a few hit on the neck from this professional executioner because there are no rule that specifically handle this kind of situation(there is the Coup de Grace rule but still).
I try to go a bit beyond the rules sometimes, and if they mention that limbs could be severed I guess it could be done or allowed by the DM.

Non-lethal doesn't mean that nothing gets cut it only means that you can't die from those injuries.

That's precisely what Coup de Grace is for. Assuming the headsman is a big burly guy with Str 14, he's doing 3d12+9 with a crit, for an average of almost 30 points. A 20th level fighter without his gear probably has a Fort save of +15 or so (base +12, Con +3), which makes the DC 40 save rather difficult.
 


I think it would be fun to have trolls reproduce only by "budding." Every year or two there is a massive ritual in the troll-village where 90% of the trolls cut off each others' heads (using whatever mechanic you feel like) and keep the heads in separate huts from the bodies. after a while, the heads all grow new bodies and the bodies all grow new heads. Hoever, growing a whole new body from a head is exhausting work (though growing a head from a body isn't) and can only be done every few years and still be assured of success.
