Reincarnate advice/warnings

The druid in our group just made 7th level. Since he now has access to reincarnate, I need to know what everyone's experiences are with the spell. What advice and/or warnings can you give me on the spell? Any problems?

The only time I've ever had reincarnate used in my experience was a joke-campaign where we were perpetually broke... IIRC, the player (possibly me) became a rot grub (ala KoDT). Of course, I seem to recall spending a large amount of time polymorphed into both a chicken and a purple mushroom... *shrugs*
