I've actually added unlimited healing outside combat to my game, with no adverse effects. I ruled that the PCs pick up wands of Lesser Vigor whenever they get the opportunity (which I'm not even bothering to charge for or track charges on) and they use these to get themselves completely healed whenever they have a few minutes outside combat. The Dragon Shaman has unlimited healing, although only to a cap of 1/2 max hp. It is a very nice ability, even if not quite the same thing.
However, if we extended unlimited healing to clerics, then we'd somewhat lose on resource counting, but I'm not convinced that it's a bad thing.

Then again, I usually only have 1 combat and maybe 2 in a given game day, which means the PCs (who are very buff and about as wealthy now as recommended for PCs 4 levels higher) almost always go into a fight with all of their resources intact. With all of that, healing fully outside combat is a negligible area to change.
