While reading Almuric by REH, there's a scene where the main character is facing ancient enemies
Hos feelings indicate an almost berserk need to kill them

I would call it revulsion and give it a +2 to hit and -2 to ac

To me it sounds more like :-

Unnatural Rage (compulsion) - Fear
Any enemy who can see this creature must make a saving throw at the start of their turn (fear effect), on fail the target must attack the creature this turn.

You could change the compulsion factor by adding bonuses or penalties to its saving throw.

You could also add other interesting riders on the effect:-

If the target fails their saving throw by more than 5 points then they must also move towards the creature.

or

If the target fails its saving throw by more than 5 points then they must charge the creature this turn but recieve an additional +1 bonus to hit and deal +2 damage, if the target cannot charge then they are dazed until the end of their turn.

or maybe

If the target passes its saving throw by more than 5 points all allies within 3 squares gain a cumulative +1 bonus to their next saving throw vs. compultion

Those are both good; if I was feeling carried away I might set it up like a whole disease progression (maybe one that advances very quickly, like every round.)

Cheers,
