OK, so making the books require use of wish or miracle. If its a +5 book does it require the 5 castings in immediate succession? The logistics of that get a little complicated.

No, you need to provide the spell once per day of the creation time, just like any other item creation. In fact, that is pretty much the whole point of making them: you can create one that gives you a bigger bonus than you have ninth-level slots available.

As to why people would make lesser books: I've always assumed that they were a work in progress, to be upgaded all the way to +5 as gold and XP supply allowed, and then read.

I would assume a +2 book was made on commission, and simply never used.

The really interesting question is why anyone would make less than a +4, since a +3 could be obtained by bargaining with an efreet.
