Instant death from one tail attack of a Nightcrawler. :D

Personally, I like a modified Massive Damage rule. 2x CON in one hit = Fort Save v. (20-CON Modifier) or fall to -1 hp. (Dying, but not dead).

Though that's really for the House Rules forum ;)
 

I had a situation where I rolled two natural ones in succession. The first was a fort save against horrid wilting, the second against the massive damage roll. And he had full HP at the start of that round.
