Scariest Monsters?

As a player, the scariest monsters are those that drain Constitution or bestow negative levels. (The last isn't nearly as scary nowadays as it used to be, but still ... ) I'll also give a shout out to the rust monster someone already mentioned, if I'm playing a tank.

As a character, I'd go with the plurality and say mind flayer. The thing about illithids is that they're this close to being something you can reason with, but that apparently tiny gap is actually a chasm miles and miles wide. One it's easy to fall into.

Have you read Robert Heinlein's The Puppet Masters? If not, than you need to.
 

Worm that Walks, from the Epic books. Writhing tangle of Meteor-Swarm casting eeeeevil.

So we're sitting there at the table, fighting an old mage buddy of ours. My character (monk) runs up and grapples said mage. Unfortunately, I have True Seeing running... GM takes me out of the room and shows me what I _actually_ see. Everyone else hears me _scream_ "OH MY GOD WHAT IS THAT THING!".

Oh come on! The Saquatch is definitely the scariest monster in D&D. So is:

* The Dire Lion.
* The Elasmosaurus.
* The Lluska.
* The Kraken (one guy was diving off the coast in the Sea of Cortez and was attacked by a Humbolt Squid! Two of them! AGGH!).
* The baby blue octopus of Australia (I forgot what it is actually called).
* Sea snakes, well no, not really.
* Great White Shark.
* Bull Shark (the thing is overflowing with Tetosterone!)
* the Worms of the Gobi Desert (something like the worm from Dune).
* Sabertooth Tigers of the Andes.
* The Dragons of the World (luckily they were all extinct*, but the tall tales of the Dragons grew into tales of beasts like the D&D dragon).
* A Mother Grizzly at full charge defending her young from you.
* A Kodiak Bear.

And the number one scariest monster in D&D is . . .

The Humano-Imunodeficiency Virus, otherwise known as AIDS.

* With probable exception of Mokliembe in the Congo and the Compsagnathus-like animals of the Four Corners.

Giant Cockroaches.
Even the little real ones creep me out. I will destroy my house chasing those little @!#$!@#$!@#.

If I saw a giant roach I think I'd drop dead right on the spot.
 

Fiends.

Immortal creatures who's only purpose in existance is to see to it that you suffer for all time. Infinite numbers, unimaginable intelect, eternal patience, Fiends are the scariest creatures in DnD. They'll hunt you in your home, kill you in the most horrible ways ever concieved, and then track down your immortal soul in the afterlife and drag you kicking and screaming down into the pits of hell, from which there is no escape. Abandon all hope, ye who face the Fiends.

Then again, if you can't beat 'em...

And how about the Neothelids from page 204 of the EPH? A GIGANTIC worm, with tentacles coming out of its mouth, spewing acid and grabbing people with its tentacles so it can swallow them whole. And that's not even bringing the powers into the equation!

This big nasty is the result of an illithid reproductive cycle gone horribly wrong, and the illithids themselves don't even want to discuss their existence. Anything that can scare an illithid is going to be cause for serious nightmares, if you ask me.
 

I find the idea of Dopplegangers pretty freaky. Perfect mimicry - yuck.

Giant Wasps. I hate wasps. Giant ones scare the bejesus out of me. :)

Nightwalker - there's something off about those monstrosities.

Mind Flayers - eating my brains - double yuck.

Dragons?
