If you want to join hong in saying that when you play your warriors swing each time you roll, plus cleaves, plus attacks of oppurtunity and what have you PLUS swing and miss some random number of times in addition to all that then yeah.

There is a passage in the PH that notes that a character will still be taking swings and defending himself when it's not his initiative. So, yeah, one attack roll can easily equal more than one swing.

How you visualize it depends on the character and the situation. A 1st level warrior probably makes nearly as many actual swings in his six seconds as a high level fighter does in his - but, at most, the unexceptional 1st level warrior is going to land one solid blow, plus, maybe a cheap-shot attack of opportunity if someone all but blunders into one of his swings while doing something stupid. The high level fighter - say with Cleave and Combat Reflexes - OTOH, may well be hitting something with just about every swing... maybe even getting in more than 1 attack roll/swing if you take Cleave a tad litterally. ;)
