Writing orders can be simple and brief, to convey basic intent.

Phase 1. Movement & Missiles

* (Clarion, the Halfling): "Rush up mound to fire on foes with Bow."

The referee also (mentally, perhaps) notes the Ogre too is rushing up the mound to control it.

* (Ogre): "Rush to top of mound."

The incline from base to top is 30 feet.
Clarion has MV 120 and is 30 feet from the base of the hill.
Ogre has MV 90 is a 10 feet from the base of the hill.
Let's assume they don't see each other (they are on opposite sides of the hill).

Phase 2. Movement & Missiles

Move is simultaneous, so the referee notes half move for all combatants at the same time.

Clarion moves 60 feet
Ogre moves 45 feet

The Ogre reaches the top first.
Clarion is still coming up the mound when she sees the hulk of an Ogre come into view and command the hill.

They are now about 15 feet away. Being within 30 feet means they are engaged in melee now. The referee also makes a ruling that the Ogre has a position of advantage. While Clarion is unable to effectively use her Bow in this Phase 2, or in Phase 4. Missiles, she can swap weapons and is now ready for melee in Phase 5. Poor thing ... luckily, the Wizard's intent was to use lighting bolt on the Ogre ....

The original guidelines made no accommodation for altering intentions/moves as they happened (as if someone were to realize mid action they wanted to do something different, as they watched the situation unfold). But let's say the referee is fine with this kind of thing, and if Clarion and the Ogre can see each other, and they never come to within melee range, and they are both rushing the hill, then Clarion or the Ogre may want to change their intentions as actions unfold. Perhaps the referee allows them Dexterity checks, or some other such mechanic to successfully do so?
