Superman, in-continuity, killed three times:
The Golden Age Superman (y'know, the 1938 guy who started all this browhaha) killed the Anti-Monitor, consciously breaking his vow for the first (and only) time.
The Silver Age Superman, in his last storyline (Whatever Happened to the Man of Tomorrow, by Alan Moore), after losing nearly all of his friends, was facing the ture Mxyzptlk (in ogre-sized demon form). He used the Phantom Zone projector on Mxy, who said his name backwards to escape to the 5th Dimension. The two forces ripped Mxy apart dimensionally, killing him. Because he had taken a life (for the first and only time), Superman entered a room with gold kryptonite (which removed his powers permanently).
The current Superman (debuted in John Byrne's 1986 Man of Steel mini) was brought to a pocket universe containing only Earth and Krypton. In this pocket universe, 3 kryptonian criminals caused a cataclysm that killed everyone but themselves and Superman. Being immune to that universe's kryptonite, he used the gold kryptonite to remove the criminal's powers. They vowed to get their powers back and escape to Superman's world and destroy it too. With no other choices left, and as the last representative of justice in that universe, Superman used green kryptonite to execute the criminals. Before Zod could die, he blamed his henchman Quex-Ul, who killed him before the green-k did. The last to die was Zaora, who offered herself to be SUperman's concubine right before she expired. Wracked by guilt, Superman developed a secondary personality that operated as the vigilante Gangbuster. When confronted with the truth of his actions, he exiled himself in space. He never killed since that episode.
This last bit was probably erased from continuity by Infinite Crisis. Dan Didio (DC's head honcho) said that Matrix (the Supergirl/Earth Angel survivor of the pocket universe) never existed in New Earth, so it's debatable if that entire storyline was deleted.
