There is no inertia in the ruleset only movement per action so I'd say yes.

Probably not, unless you're using a flat world. Remember, a person on the surface of the earth is moving about 1000 miles per hour in the direction of rotation (at the equator). If you teleport to the opposite side of the earth and momentum is conserved, then you will be moving at 2000 miles per hour opposite the direction of rotation (relative to the local surface). Not good.

I would say sometimes :)

It is driven by the needs of the magic.

In a classic bottomless pit trap, where there is a teleport circle at the bottom which pings you up near the top again, momentum is conserved so that you have the sensation of falling forever.

For teleport spells, I would judge that those who created them had the wit to create magic which didn't conserve momentum so that at the end of a teleport you are always considered to be at rest and with the appropriate potential and kinetic energy. After all, if someone goes to the effort of creating a spell, why not create one which does what you want safely?

Yknow, I always assumed the arcane elite were constantly trying to make safe spells and kept coming up with explosions.... so they took up adventuring.
