To me this comes down to letter of the rule versus spirit of the rule.

The Eladrin entry does not state that Eladrin never take falling damage. It says they get a move action, 5 square, teleport. I'd simply rule that the player could not teleport the last five feet of any fall to negate damage and leave it at that.

I'd go falling damage for however far you've fallen, not counting any teleportation distance. Mind you, how would teleporting in mid fall work? a readied action "I ready an action teleport as soon as I have fallen 5 ft" Can you ready an action to key off your own falling, which takes place during your move action?

A move action is too slow to save you from most falls. And you can't jump directly into a teleport, because you can't insert a move action into the middle of another move action. Even double move only works if they are the same kind of move action.

So whether teleports conserve momentum or not, it won't make much of a difference to the Eldarin. I'm personally leaning towards not conserving momentum, just because it seems to match up better with the way the rules work.

(Those ritual portals, on the other hand, might work differently from normal teleportation movement.)

edit #2: Also remember that readied actions are immediate reactions, which means you can't use them on your turn.
