Add in re-roll initiative each round. If has some drawbacks, but also a lot of benefits.
It can make effect durations harder to track, thought I will argue don't bother, use the duration that is easiest to track (use initiative count or actor turn). It makes players pay attention, and they always have to be ready at the start of a round.
