Black Dougal was the first character I ever played. He got killed by crab spiders in the Haunted Keep from the Basic Set (which, IIRC, was about 3 or 4 rooms in.)

I think it is just his fate.

I thought that was Black Leaf .

Black Leaf, too. Black Dougal fell to a poisoned needle trap in the example of play in the Erol Otus basic rulebook (pink box).

It appears that having "Black" as your moniker shortens your lifespan.
