I take it books that could be Player's Handbook replacements were deliberately left out, so that the list consists only of books that can be used as D&D supplements? That leaves out some great stuff, like Arcana Evolved, Iron Heroes and Grim Tales.
In any event, I'd like to add to my vote an honorable mention to Character Customization by tiny PDF-only imprint Throwing Dice Games, even though they never did release the long-promised 3.5 version.
Of the books listed here, Occult Lore and Torn Asunder I didn't like at all. Some of the others, like Shackled City, got the job done, but didn't strike me as spectacular.
