The bigger they are, the harder they are to kill

The idea for the new concepts here I think are trying to address a real problem, not just trying to bring real life into D&D(wich never realy works anyway, like bringing real life into monopoly).
I think it is horrible that Dragons have to use hit and run tactics, and that by the time parties face most giants they are just their level orc or ogre(an eggshell which deals alot of damage)
A Dragon should be able to land and fight in melee if it so choses take on 4-6 men wich are ants to him and fly away with a snack in his belly. When it gets to the point and you see a monster with 120 or so hit points and know it wont last more than 2 rounds, without using environment and lots of tactics is just crazy. Higher level character dish out an overlypowerful amount of damage.
Think about it, you are playing your weekly game and the DM Throws a boulder at the party from a couple giants up on a cliff, are you as concerned when he says that or when he says you get sniped by a equally capable archer to your groups and a wizard unleashes a fireball at you, you look up and see another party of equal capacity on top of the cliff.
I think the others and I here just want to bring back some awe and power to the bigger than life creatures in D&D, the hit points don't do it, a d12 give me a break one point over per HD than the main fighter types and the same as one of them.

Sir Brennen has it. This is a double slap on the wrist for melee types - when one already works.
Add the increased CON for large creatures, as someone already pointed out, and we have an even stronger argument that the system already addresses this problem.

But that dragon has more of those d12's, a higher CON, higher STR and bigger base damage - all due to its size.

And 1st level fighters are ants to that same dragon. It sounds like you have more of a problem with the way damage and hit points increase for PCs as they gain levels, and possibly with the level concept itself, than a need to make size differences more believable.

To that end, I recommend playing some higher level GURPS heroes against that system's dragons if you want the sense of danger you seem to be looking for, Paraxis.

Steal from other d20 sources.

All monster HD are now 1d4+ a static bonus. The total max is still the same but the min and average are tons better.
Giants now get 1d4+4 hp (plus Con bonus) per HD.
Dragons get 1d4+8 + Con hp per HD.
Fey get 1d4+2 + Con hp per HD.
That will change some perceptions in a big hurry.

If you're willing to change systems, check out True 20. Big monsters are TOUGH! They're also so strong that getting hit by one is likely to be extremely bad news, no matter what level your players are.
