I played in a game once where the forces of neutrality were at odds with each other. One was for moderation the other for balance. They occasionally waged war by proxy, which is how the PCs were involved. I also seem to recall some magic item in Baldur's Gate that did damage proportional to how far away the opponent was from true neutrality.
If you wanted to go another route, you could have a book that was all about how to DM moral relativism. Paladins killing captured kobolds, arch liches having a point of view, etc.
