A 6ft human with a longspear standing behind a 3ft halfling has cover from the halfling, but creatures in front of the halfling do not have cover from the human.
That only occurs IF the attacker is closer to the low cover than the being being attacked.

Thus a huge giant may be able to invoke that rule when making an AoO at a medium critter closing in at 15' away, but the troll can't use that for the foe only 10' away.

Considering there has not been errata, FAQ, web article, reprinting or a new edition revising this subject, the tread may be dusty, but the issue has not changed much.
