I've had a few in my FR campaign. The last three were: a mature adult green dragon, Lolth, and the albino drow chick from City of the Spider Queen.

In my current Lord of the Rings game I've got three BBEG's: Sauron (of course), Saruman of Many Colors, and Carangul the Red Sorcerer (he's an evil black Numenorean magician and thrall of Sauron in Angmar. Right now he's working with Saruman to dig up Numenorean relics in Annuminas to use in the war effort. Kind of Raiders of the Lost Ark meets The Lord of the Rings).

Lilybo Orge with silver crown
Lilybo Troll with gold crown
Lilybo Halfling Thief
Lilybo Stone Giant with platium crown.
The halfling is dead. However a year or so before he was killed he tried to steal a magicial crown under construction. He nearly made it. The crown broke into it component parts and absorb his mentality. So each creature wearing thinks it is Lilybo the head of thieves guild in a local city.
The orge was killed by the party and one member occasionally wears the silver crown.
Ego checks are made to see who in control of the body.

It was created with the help of numerous ENWorlders. You can see a picture of him in this thread.

I love it when someone asks for help and we get to see the end product! :)
 

Well, my games are still ongoing, and some of my players read this board (but not enough for me to bother with spoiler tags), so I'll only say a few things.

The Coming Night (face-to-face):
The Wraith-King - Basically a Witch-King knock-off who had enslaved an elven kingdom. Defeated, but nto killed, by the party, he's getting hsi vengence on.

Lord Blackmere - A human noble leading the major force to oppose the party in their kingdom's civil war. Not necessarily evil, just ambitious... and a bit morally questionable.

Dor Ulm (Defeated) - The big bad evil nation of the first story arc. Their armies were defeated by the party's kingdom's armies with help of some questionable allies (one fo the factors that lead to their kingdom's civil war).

Unknown #1 - Some force is hindering clerics from regaining their spells easily, except for those clerics who worship The Lady - a newly-come goddess in a world that previous had no gods as such. The party suspects the Lady, but she and her clerics keeps doing so much good for the world, darn it... how could they be evil? :)

Unknown #2 - Something has caused the dwarves of the Grey Peaks to suddenly collapse all the entrances to their underground kingdom, and they ain't talking. Nobody knows what's going on there.

Unknown #3 - That would be telling :)

The Legend of Aerilis (PbeM):

Khelrija (Killjoy in Common) - An ancient devil from a group known as the Tel'Ramar - devils looking to destory the world. Recently freed from his prison and resuming his old duties.

The Ebon Feather - A cult interested in undeath.

U (An initial) - Unknown fellow seeking an evil artifact that the party recovered, and may have been sneakily stolen just before its destruction.

Not really BBEG, but fun recurring (and eventual foes):

Guell Shadowpad - former thiefly mentor of the the party's bard, now has a hate on for her since she caused him to be caught.

King Mantis - Proof is lacking, but it seems as though he conspired to assassinate the old king and his heir, and then took the throne. Also outlawed the order one of the PCs is trying to join.

O'mar Mantis - Son of the new King, but doing his own thing. Kidnapped the love of one of the characters and betrayed his church.
