D&D 5E The Fighter Class and superiority dice

I'd probably give the target a saving throw (in the round after a successful grapple check, assuming the target doesn't escape) to avoid the broken bone. I wonder if that sort of thing would throw a wrench into the system - if you should go through HP like [MENTION=907]Staffan[/MENTION] suggests. My gut feeling is that it would still work okay.
I think the philosophy of 5e is that the DM should trust his gut, and that the system treats wrenches as lubricant.

My reasoning is that your hit points are your defensive skills (among other things). Someone with a lot of hit points left should be able to avoid a broken arm from someone grappling him, just like he should be able to avoid a broken arm from someone hitting him with a hammer.

Remember, 5e is a system where even a paralyzed or unconscious creature still has their hit points as a defensive measure - attacking a paralyzed or unconscious creature has advantage and is an automatic crit in melee (if you hit), but it's still going to take a while to kill someone with 100 hp that way.
