Amulet of Mighty Fists:
This amulet grants an enhancement bonus of +1 to +5 to attack and damage rolls with unarmed attacks.

Unarmed Damge when charging 20 * (1 (Base) + 1 ( Mantis Leap) + 1 (1 Boots)) = 60 (so where does the 90 come from?) (Stacking multipliers don't multiply they add)

Lycanthrope stat modifiers only apply in hybrid form. So I think your Str should be lower. Also if you're a natural lycanthrope you have no choice about takeing the basic Hit Dice. Which makes the ECL equal to the HD. If you're not you can't be a half-dragon as half-dragons can't catch lycanthropy.
 

I'm not stacking multipliers for the same thing.

Base is 4d6(base unarmed damage) + 20(strength) + 5(enhancement)

Mantis leap doubles strength mod to damage: now 4d6 + 40 + 5

Sandals of Tiger's Leap: These sandals allow the wearer to make devastating flying kicks. The wearer must have 5 ranks of Jump or Tumble to use them. On a charge, she can perform a flying kick. Treat this as an unarmed attack that deals double normal damage.

Damage now is (4d6 + 40 +5) * 2 or 8d6 + 80 + 10

I'll deal with the template crunch in the morning.
 

2X your str mod with a feat and 2x it with those boots is a stacking multiplier, therefore its times 3. Unless you thing that a maximised empowered fireball does automatic 90 points of damage. Or an energy admixtured empowered fireball does 30d6 damage.

And I'm leaning on saying only the bite and the first unarmed attack gain the double damage. Since those boots certainly were not designed with that rather unique PrC ability in mind.
 




The half-dragon template usually costs 10 levels in a prestige class (i.e., dragon disciple). It sure seems cheap in this game though.
 

With Kal's vetoes on some important aspects of the monk, I will more than likely go back to the druid unless there are some ideas floating around out there to help him out...
