The monster you fear most

My players fear ... any "named" devil prince (not bob)

Duh - cause Bob is a demon. A dretch in-fact.
They would fear him if they ever met the 11th level sorcerer version of bob
(inspired by from dretch to demon lord) riding his marble elephant!

maybe fear is not the right word....

its a rainbow sparkly dragon that is more powerful than you can even imagine

the wrymlings are born the size of elephants

oh...and when I dropped one of these infront of a power group (30+ levels, it was ridiculous!) they laughed

Rust Monsters. Haven't seen one in over 20 years, but when I do, I'm sure I'll run - just like I did when I was 12 years old.
 

How about combining stirges and rot grubs? There are several worms that mosquitos spread.

Have any of use used disease bearing stirges?
 

Intellect devourers. They hollow out your head and take up residence as your brain.

Penaggalans might be a close second. And stirges sort of bother me as well.
 

At present, my group has a real problem with wights. Something about insubstantial, phasing and flight really bugs the heck out of their configuration and they've nearly TPKd twice when dealing with them.

In terms of my own experiences, I'll say that Ogre Mages are nasty and need to be dealt with quickly.
