True20 Form Fillable Sheet

I have just uploaded True20 Form Fillable Sheet to the downloads area.

A sheet made in openoffice due to lack of form-fillable sheets on the internet. Contains more than enough info and space to make a character.

Thanks, but when I tried to fill the form, it repeated the same value everywhere. For instance, when I wrote "Expert" in Role/Level, it copied the role to all the fillable boxes in the form. If I write 1 in Strength, it writes 1 all over the place, etc. It's as if all the fields have the same label. :-(

I'm using Adobe Reader XI on Windows 8.1.Should I try with Reader DC or another program?
